About Us Queen's North Hawaii Community Hospital - is a 35-bed rural acute care hospital located in Waimea (Kamuela), Hawaii Island. Scope of Service We provide intermittent quality care for homebound patients, of all ages, by offering a network of services to meet the emotional and physical needs of both patients and families. - -We provide Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, as well as Certified Home Health Aide care, and Medical Social Worker services. - Our service area includes Hilo, Hamakua, North and South Kohala, North Kona, Keaau and Volcano. Responsibilities I. JOB SUMMARY/RESPONSIBILITIES:--- Under the general supervision of the Kohala Home Health Manager or the designee, assesses, plans, implements, coordinates, monitors and evaluates options and services required to meet the needs of the patients to promote quality cost-effective care by providing assistance to staff. ---- Ensures that specific outcomes are achieved and that variances from established and accepted parameters are evaluated and addressed as necessary. ---- Is knowledgeable in community health nursing and rehabilitation concepts and is able to work independently as well as communicate effectively with clients, staff and physicians.--- Provides lead direction to professional, technical and support staff, and provides case management services for patients.II. TYPICAL PHYSICAL DEMANDS:--- Essential: - finger dexterity, seeing, hearing, speaking.--- Continous: - sitting, static gripping of an object for prolonged periods.--- Frequent: - walking.--- Occasional: - standing, stooping/bending, climbing stairs, walking on uneven ground, lifting and carrying usual weigh of 1 up to 5 pounds, reaching above, at and below shoulder level, frequent gripping of an object.--- Operates computer, calculator, telephone, fax, printer and other business office machines. - Operates therapy and medical equipment.III. TYPICAL WORKING CONDITIONS:--- Not substantially subjected to adverse environmental conditions.--- Exposure to inside and outside environmental conditions.--- Some exposure to communicable diseases and/or body fluids; some toxic substances and other conditions common to a homecare environment.--- Requires access to transportation to assist with client services. -IV. MINIMUM QUALIFICATIONS:A. EDUCATION/CERTIFICATION AND LICENSURE:--- Bachelor's Degree preferred.--- Current Hawaii State license to practice as a Registered Nurse, Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SLP).--- Valid U.S. driver's license, vehicle and vehicle insurance as required by law.--- Meets and complies with qualifications set forth in QNHCH fleet/Motor Vehicle policy.--- Current BLS/HCP certification.B.
