Hale Kupuna in Koloa, Hawaii is looking for a MDS Coordinator to help us with sourcing, locating, and recruiting talented, best qualified, and culture-add candidates who embrace Ohana's mission and values. Ensures timely completion of accurate MDS assessments, care plans and electronic MDS transmission for residents. Coordinates the Interdisciplinary Team (IDT) team in development of RAI process in accordance with federal and state regulation. Reviews, edits, corrects, and ensures assessments are signed appropriately signifying completeness and accuracy of all MDS, CAA's, Care Plans and care conference notes. Appropriately maximizes reimbursement and resident-centered quality of care delivery based on best-practice standards of care and accurate capture of resident acuity and complete documentation of services provided. In collaboration with the IDT and community leadership, provides direct clinical and reimbursement oversight through a case management model to ensure reimbursement maximization and high quality of care delivery from pre-admission through discharge.

Shift: Full-time, Non-exempt, Day 7:00 am - 3:30 pm.

Duties include but are not limited:

  • Supervises and managesthe restorative nursing program.
  • Collaborateswith theStaffDevelopment& Infection Prevention Coordinator and ensures appropriate coding on MDS as applicable.
  • Coordinates and conducts the development, completion and transmission of the resident assessment in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation and completion of CAAs and Triggers.
  • Ensures that all assessments are completed, locked and transmitted in a timely manner.
  • Ensures that the appropriate professionals are involved in the assessment and that the care plan identifies and addresses the needs of the resident, includes measurable goals, timeframes, and resident-specific approaches including delineation of the professional discipline responsible for each element of care.
  • Coordinates pre-admission reviews and admissions into the facility in conjunction with the IDT.
  • Conducts or coordinates the interviewing of each resident and/or family, based upon resident's / family's ability to participate, as well as involved caregivers for the resident's assessment. Interviews include but are not limited to pre-admission, day of admission, ongoing as necessary. Conducts comprehensive chart review and assesses resident as needed to acquire updated information for MDS, CAAs, and Care Plan completion. Ensures necessary assessments and documentation such as physical restraints, falls, psychotropic medication monitoring, monthly nursing summaries are completed in the medical record and reports problem areas to nursing management.
  • Ensures the initial resident assessment and comprehensive care plan is completed within required OBRA and PDPM/PPS timelines.
  • Evaluates each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
  • Ensures that the complete resident assessment is conducted within fourteen (14) days of a significant change in the resident's condition.
  • Coordinates the review and revision of the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised in accordance with state and federal guidelines.
  • Facilitates the IDT Medicare meeting to include appropriate capture of RUGs; monitoring of all Medicare Part A and Part B residents; monitoring of 30, 60 and 100-day windows and ensuring appropriate level of care process is followed.
  • Reviews facility CMI report monthly.
  • Coordinates, in conjunction with Social Services, the scheduling of resident care plan meetings and family conferences.
  • Actively participates in discharge plans from time of admission by coordinating efforts with IDT, practitioners, residents, and responsible parties.
  • Attends Safety, Management Team and Staff meetings as required.

Requirements:

  • Graduate of an accredited school of nursing.
  • Minimum of 1 year of professional nursing experience required.
  • Knowledge of medical terminology and abbreviation required.
  • Current license as a Licensed Practical Nurse or Registered Nurse in the State ofHawaiirequired.
  • Maintain active RAC-CT through the American AssociationofNurseAssessment Coordination (AANAC).
  • Clinical experience and /or case management experience preferred.
  • Familiarity with computers and word processing programs required.
  • Experience in geriatrics and current nursing standards of practice preferred.
  • Experience as an MDS Nurse preferred.
  • First Aid and CPR preferred. (can be done in-house).
  • Must have current medical/health screening including 2 step TB clearance (can be done in-house).
